Tattoo Establishment License
Tattoo businesses in Florida cannot operate without a license. To apply for a tattoo establishment license in Lee County, Florida, you must submit the following items to DOH-Lee.
- Download and complete the Application for Tattoo Establishment License. Email the completed application to LeeEH@flhealth.gov.
- Register the business name with the Florida Department of State.
- Contact the city/county to check if you need a business tax receipt.
- Pay a $200 fee. You can pay online by visiting MyFloridaEHPermit.com or select “Pay Invoice. ”Apply for and receive a Biomedical Waste Generator Permit. This is required for conventional and cosmetic tattoo businesses.
- The annual fee is $150 ($85 state of Florida fee, plus a $65 Lee County fee).
- Pass a DOH-Lee inspection in compliance with sections 381.00771 - 381.00791, Florida Statutes, and Chapter 64E-28, Florida Administrative Code.

Having anyone tattooing in a tattoo shop without a valid tattoo artist license, subjects the business to a fine and cancellation of their tattoo establishment license.
